Improving Adherence to Recommendations

Reducing a diabetic patients HgbA1C levels through open communication, support, and personalized management

When traditional methods for communicating a diabetic management plan to a patient proved unsuccessful, a more personalized approach was implemented. The patient received bi-weekly phone calls from her clinician to collaborate on her care plan and ensure that the patient was adhering to her medication and dietary requirements. The patients family mobilized to be more involved as an at-home care team and encourage the patient to take ownership of the management of her diabetes.

As a result, this patient experienced a dramatic decrease in their fasting sugars and A1C level within three months.
